This program is for 12 months. It includes a glucose monitoring pack delivered to you at no cost. Included in the pack is a glucometer which digitally records your self-measured blood glucose data onto a digital health platform. This program also provides support and coaching from a professional health team and an integrated health report every second month after registration.
